What is your financial policy?

Patients are expected to understand their health insurance that is in effect at the time of appointment. Most insurance plans now have a co pay and many are high deductible plans. Payment is expected at the time of service. Payment may be made by cash, check, money-order, MasterCard, Visa, Discover or American Express. OAH will expect outstanding balances to be paid prior to follow up appointments.

What insurance do you accept?

We participate with the following insurances : Aetna, Anthem Blue Cross Blue Shield, Cigna, ConnectiCare (unless you purchased it on the Connecticut Exchange), United Healthcare, Oxford, HNE Health New England (PO and Managed Medicare only), traditional Medicaid and Medicare. We also participate with most Workers Compensation PPO's.

We are currently Out of Network with Harvard Pilgrim and Multiplan PHCS.

The only Connecticut Marketplace (Exchange) product we participate with is Anthem Blue Cross Blue Shield.. We do not participate with ConnectiCare's Exchange product.

We participate with the following Medicare Advantage plans: Aetna, Anthem Blue Cross Blue Shield, ConnectiCare, and United/AARP.

How do I get a refill on my prescription?

Whenever possible, prescription renewals should be called in two days before your supply is exhausted. Please have the telephone number of your pharmacy available. All prescription refills must be called in during working hours of 9:00AM to 4:00 PM. Due to the fact that our physicians cover for each other during nights and weekends, it is imperative that you have your prescriptions filled by your OAH physician during the normal business hours. Covering Physicians will not refill prescriptions without prior consent from your OAH surgeon. The Urgent Care Centers will not fill prescriptions as part of their routine service.

What are your office hours?

We are open between 8:30AM to 4:30 PM on Monday through Friday. We have coverage 7 days a week 24 hours a day for emergencies. There are 2  orthopedic only Urgent Care Centers available between the hours of 4:30 PM to 8:00 PM Monday through Friday and Saturday/Sunday  9:00 AM to 1:00 PM. These are located at the 345 Western Boulevard, Glastonbury and 499 Farmington Avenue, 3rd Floor, Farmington. No appointment is necessary.

What paperwork should I bring?

Be sure to bring a photo ID, your insurance card, referral if needed, and workers compensation authorization if needed.
