Medical History

To ensure that you have the safest and most effective medical care possible, it is important that you provide a complete medical history to your physician.

If you agree, prior to your visit, we will send you a link via email that will allow you to complete your medical history at any time, at your convenience. Your encrypted medical history will then be transmitted to your physician electronically. If you complete your history prior to your scheduled visit, then we would ask that you arrive just 5-10 minutes early so that our staff can complete and verify your registration information.

If you would prefer to complete your medical history in our offices prior to your scheduled visit, then plan on arriving 20-30 minutes before your scheduled visit to complete the medical history and registration process.

We strive to be on time and to provide effective care to our patients. Having all of your “paperwork” out of the way before you scheduled appointment helps make that a reality.
