Prepare For My Visit
It is important to bring the following items each time you visit our office:
- A photo ID
- Your insurance card
- A referral, if needed
- Workers’ compensation authorization, if applicable
- Please bring a method of payment for unmet patient responsible balances, deductibles and co-pays
If you have any x-rays or tests, please bring these results to your visit. We prefer to receive x-rays on a flash drive.
We encourage patients to provide us with an email address when booking an appointment so we can send you an online form to complete prior to your visit. This will save you time and provide us with valuable information about your health.

Complete your medical history
To ensure that you have the safest and most effective medical care possible, it is important to provide a complete medical history to your physician. We encourage all patients to save time and complete your medical history online prior to your visit.
If you agree, prior to your visit, we will ask for your email and will send you a link that will allow you to complete your medical history online at any time, at your convenience. Your encrypted medical history will then be transmitted to your physician electronically.
If you complete your history prior to your scheduled visit, we would ask that you arrive just 5-10 minutes early so that our staff can complete and verify your registration information.
If you would prefer to complete your medical history in our offices prior to your scheduled visit, plan on arriving 20-30 minutes before your scheduled visit to complete the online medical history and registration process.
