Prepare For My Visit

It is important to bring the following items each time you visit our office:

• A photo ID
• Your insurance card
• A referral, if needed
• Workers’ compensation authorization, if applicable
• Please bring a method of  payment for unmet patient responsible  balances, deductibles and co-pays 

If you have any x-rays or tests, please bring these results to your visit. We prefer to receive x-rays on a flash drive.

We encourage patients to provide us with an email address when booking an appointment so we can send you an online form to complete prior to your visit. This will save you time and provide us with valuable information about your health.
